## Central hypothesis

Two exposures with the same cumulative dose can differ if one is delivered in brief peaks and the other continuously. This paper asks when peak concentration, receptor adaptation and recovery windows matter more than the time-averaged amount. The concept is general; claims must be chemical and tissue specific.

## The biological route

Toxicity may depend on maximum concentration, area under the exposure curve, saturable metabolism, repair kinetics and inflammatory thresholds. Endocrine and immune pathways can show adaptation, priming or desensitization after repeated pulses. Short peaks can be more dangerous in some contexts and less consequential in others if rapid clearance prevents accumulation.

## An illustrative scenario

Two chemical regimens can deliver equal total dose yet produce different peak concentrations. For a rapidly cleared compound with saturable toxicity, peaks might be decisive; for another, cumulative exposure may dominate. It would be a category error to apply one result automatically to dietary oxalates, pharmaceuticals, vaccines and industrial solvents. Mechanism requires an identified substance and measured internal exposure.

## Alternative explanations and limits

Equal integrated exposure does not imply equal internal dose, especially when absorption and elimination differ. The “pulse” framing cannot be invoked generically to assert harmful effects from vaccine schedules, food ingredients or medicines without identifying the substance and relevant exposure kinetics.

## What would change the conclusion?

Discriminating experiments match total internal exposure while varying peak height and interval, then measure named tissue endpoints. If outcomes depend entirely on total dose, the pulse-specific mechanism is not needed.

## The distinctive prediction

Two exposure shapes should be compared at equivalent internal cumulative dose, not merely equivalent administered amount. That distinction is especially important where absorption or metabolism is saturable.

## Clinical interpretation

The takeaway is a demand for better time-resolved exposure models rather than arbitrary recommendations to avoid repeated everyday exposures.
