## Central hypothesis

Host defense does not simply sterilize tissues; it coordinates tolerance, removal of damaged cells, epithelial maintenance and responses to resident microorganisms. The gardener metaphor proposes that health depends partly on regulating an ecosystem rather than eliminating all microbial or inflammatory activity.

## The biological route

Mucosal immunity, barrier-derived signals, antimicrobial peptides and regulatory lymphocytes help sustain coexistence with commensals while restricting invasion. Macrophages clear apoptotic cells and influence remodeling. Excessive or deficient activity may both impair tissue function. A local ecosystem thus requires selective response rather than globally strong or weak immunity.

## An illustrative scenario

A healthy gut normally contains many resident microbes that neither need to be eliminated nor should be described as sterile invaders. At the same time, invasive pathogenic infection can require decisive antimicrobial treatment. The ecological view becomes useful when it distinguishes coexistence, containment and active disease using real microbiology. If it labels every antimicrobial intervention as environmental damage, it becomes ideology rather than medicine.

## Alternative explanations and limits

For many infections, microbial killing is decisive; for immune-mediated conditions, strong anti-inflammatory treatment can be life-saving. Gardening language can become a misleading argument against antibiotics, vaccines or immunosuppression if it ignores pathogen-specific realities.

## What would change the conclusion?

The scientific task is to define which cell populations, organisms and tissue functions are being maintained and to demonstrate that targeted ecological changes improve outcomes. Diversity alone is not necessarily beneficial; a pathogenic community can be diverse.

## The distinctive prediction

An ecological interpretation is useful when it predicts selective tolerance toward commensals alongside robust control of invasion. If it cannot tell beneficial coexistence from dangerous infection, the metaphor is clinically empty.

## Clinical interpretation

This perspective can enrich understanding of mucosal health while preserving urgent treatment for sepsis, invasive infection and genuine autoimmune or inflammatory injury.
